What you’ll do
- Provide comprehensive preventive and chronic disease care; minor office procedures
- Deliver core women’s health services (paps, contraception counseling/implants)
- Collaborate with care coordination to meet quality and access goals
Schedule
- Mon–Fri 8a–6p; 4 clinic days, 1 admin day per week; no weekends
Compensation & benefits
- Base salary $250,000 DOE
- $18,000 combined sign-on bonus
- Federal student loan repayment eligible (FQHC)
- 4 weeks PTO + 11 paid holidays, CME $2,500/5 days
- FTCA malpractice, Medical/Dental/Vision, 403(b) with 3% match

Elkhart, Indiana—commutable to South Bend and Notre Dame; affordable Midwest living with easy regional travel.
