Facility Location: Keizer, Oregon 97303

Assignment Dates: As soon as possible - Ongoing

Schedule/Shifts: Monday to Friday, 8 AM to 5 PM (FLEXIBLE schedule and open to full or part-time candidates)

% inpatient/outpatient: 100% outpatient

Patient Ages: All ages

Patient Volume: Varies day to day based on procedure mix

Procedures: General dentistry, composite, CEREC, molar endo, oral sedation, and dentures

Support Staff: Front Office, 3 dental assistants, 3 hygienists, 1 other dentist

Type of Charting: Open Dental

Requirements: Oregon dental license, DEA registration, and BLS

Reason for Need: Growth, looking for locums or long-term

Credentialing Timeframe: 1 week

Pay Rate: Negotiable
