Function: General Family Dentistry

Facility Location: Perry, OK 73077

Assignment Dates: September through November, Monday-Friday

Schedule/Shifts: M-TH 8-5, F 8-1

% inpatient/outpatient: 100% outpatient (up to 40% Medicaid patients)

Patient Volume: 10-20

Procedures: General dentistry, mainly adults

Support Staff: Front Office, 2 dental assistants, 1 hygienist

Type of Charting: Open Dental

Requirements: Clear and active OK license, DEA, BLS (must be enrolled in Medicaid to be considered)

Reason for Need: Gap coverage

Credentialing Timeframe: 1 week

Pay Rate: Negotiable
