Facility Location: Tooele, UT 84074
Assignment Dates: Asap, 4 weeks
Schedule/Shifts: 4 days/week
- Tues: 9a-5p
- Wed: 9a-5p
- Thur: 8a-4p
- Fri: 8a-2p
% inpatient/outpatient: 100% outpatient
Patient Volume: 15 ppd avg.
Procedures: Exams, Crowns/Bridges, Fillings, General Dentistry
Support Staff: Front office staff, 5 Dental Assistants, 2 other hygienists
Type of Charting: Dentrix and Dexis
Requirements: UT License, BLS
Reason for Need: The Dentist is recovering from an injury
Credentialing Timeframe: 1 week
Pay Rate: Negotiable
