A well-established community health organization in Northern Indiana is seeking a Family Medicine Physician (MD/DO) to join its outpatient primary care team.

• Approximately 18–25 patients per day

• $4,500 annual CME/professional allowance

The Practice

Provide broad-spectrum Family Medicine across the lifespan, including preventive care, acute visits, chronic disease management, pediatrics, adult medicine, geriatrics, and women's health.

The organization uses a collaborative, multidisciplinary care model supported by care coordination, case management, chronic disease programs, and other clinical resources.

Qualifications

• MD or DO
• Family Medicine residency-trained
• Board Certified or Board Eligible
• Eligible for state medical licensure
• New graduates are encouraged to apply
