A well-established community health organization in Northern Indiana is seeking a Family Nurse Practitioner to join its outpatient Primary Care team.

This is a strong opportunity for an experienced NP who enjoys broad-spectrum Primary Care, team-based medicine, and serving a diverse patient population.

• Approximately 18–25 patients per day
• 5 weeks combined PTO

Clinical Scope

Provide comprehensive Primary Care across the lifespan, including:

• Preventive and wellness care
• Acute and same-day visits
• Chronic disease management
• Pediatric and adult Primary Care
• Geriatric care
• Women’s health

Qualifications

• Licensed or license-eligible Nurse Practitioner
• Family Medicine / Primary Care background
• Approximately 3 years of relevant Primary Care experience preferred
• Strong FQHC or community health experience may be considered with less tenure
• Comfortable caring for a diverse and medically complex patient population
