Family Nurse Practitioner - Outpatient Primary Care
South Bend, IN | up to $140,000 base + up to $10,000 sign-on
Join a mission-driven, team-based community health center (FQHC) providing comprehensive primary care without inpatient call or weekends. You’ll practice in a PCMH model alongside MDs/NPs, RNs, and MAs with embedded behavioral health and care coordination.
What you’ll do
- Provide full-spectrum outpatient primary care
- Perform office-based procedures within scope; coordinate referrals as needed
- Document in Athena EMR; contribute to quality goals and team huddles
- Collaborate with a supportive team of 5 NPs
Schedule
- Monday–Thurs, clinic hours 8a–6p with one late evening to 8p
- No weekends; nurse triage handles after-hours calls
Compensation & benefits
- $115k–$140k base (experience-based) + up to $10k sign-on (may be used for relocation)
- Federal malpractice coverage (FTCA)
- CME days + allowance
- Medical/Dental/Vision, 403(b) with 3% matc
- 4 Weeks PTO + 11 holidays
